Enrollment Forms & Information
A registration fee is due at time of enrollment.
Upon enrollment at Seven Oaks Academy, the following information must be on file before leaving your child in our care:
- Registration packet completely filled out (emergency/health information, contract and Seven Oaks policies).
- Copy of current immunization record (signed by physician).
- Va. State physical form (complete and on file within 30 days after enrollment) Public school copy if applicable.
- Proof of Identity (birth certificate or proof of birth from hospital for toddler and preschool. Public school age need to turn in physical). Custody papers (if applicable).
It is your responsbility to update these records as needed to insure proper information, which protects your child.
